Pathological narcissism can be expressed in temporary reactions and specific traits, or in a stable, enduring narcissistic personality disorder, NPD. Both pathological narcissism and NPD can co-occur with areas and periods of high functioning, competence, and sense of agency with goals, motivation, directions, and even outstanding contributions, or alternatively with intermittent qualities and capabilities—in specific, social affiliations or interpersonal closeness. When the level of pathological narcissism is less severe, triggered in certain situations, or limited to a set of specific character features, it is referred to as narcissistic pathology or narcissistic traits. The diagnostic term NPD refers to a stable long-term personality functioning that meets the DSM-5 section II or III criteria for NPD or any other comprehensive diagnostic description. Independently of the level of severity, pathological narcissism can either be overt, striking, and obtrusive, or covert, internally concealed and unnoticeable to others.
