I believe that OCD -- no matter the content of obsessions -- is a coping mechanism to distract from deeper distress. The key is to find that root source of distress. ERP only goes so deep -- it addresses the OCD behavior, which is necessary in severe cases, but doesn't necessarily resolve the root cause.
