Living with chronic pain or illness can affect far more than your physical health—it can change your identity, your relationships, your sense of safety, and your hopes for the future. With a strong medical background and years of training and experience treating chronic pain and chronic illness, I offer a compassionate, nonjudgmental space for making sense of these experiences and finding actionable ways to move forward. My approach is collaborative and individualized, combining evidence-based psychological treatment with an understanding of the complex relationship between the brain, body, nervous system, emotions, behavior, and symptoms. Whether you are struggling with chronic pain, symptoms that persist despite medical treatment, illness-related anxiety, medical trauma, PTSD, sleep problems, or trying to adjust to a life that has changed, therapy can help you feel more capable, hopeful, and empowered to build the life you want.

Treatment Approach

  • Acceptance & Commitment Therapy (ACT) Helps people accept difficult thoughts and feelings instead of fighting them, while committing to actions that reflect their values. It blends mindfulness with practical behavior strategies.
  • Biopsychosocial Therapy Takes into account biological, psychological, and social factors in understanding mental health. It looks at the whole person, not just symptoms.
  • Cognitive Behavioral Therapy (CBT) One of the most widely used approaches, CBT helps people identify unhelpful thoughts and replace them with healthier patterns. It is effective for anxiety, depression, and many other concerns.
  • Cognitive Processing Therapy (CPT) An evidence-based treatment for PTSD that teaches clients to reframe unhelpful thoughts about trauma. It helps reduce guilt, shame, and fear.
  • Mindfulness-Based Cognitive Therapy (MBCT) Combines mindfulness practices with CBT to prevent depression relapse. It helps people notice thoughts without judgment and respond more calmly.
  • Motivational Interviewing (MI) A collaborative, client-centered approach that strengthens motivation for change. It is especially effective for addictions and health behaviors.
Approach Description: My approach to therapy is collaborative, individualized, and grounded in the understanding that psychological well-being is closely connected to the body. I do not believe that there is one therapeutic approach that is right for everyone. Together, we will identify the patterns that are keeping you stuck and determine which strategies are most likely to be helpful for your particular concerns, circumstances, and goals. I draw from several evidence-based approaches, including cognitive behavioral therapy (CBT), trauma-focused therapy, mindfulness-based strategies, and Cognitive Behavioral Therapy for Insomnia (CBT-I). Therapy may also incorporate approaches that focus on the relationship between the brain and body, as well as helping you figure out what to accept vs. what can be changed. For those with persistent pain or other symptoms that may be influenced by nervous-system sensitization, I may incorporate principles from Pain Reprocessing Therapy (PRT) and other neuroplastic and mind-body approaches. These approaches explore how the brain and nervous system can contribute to the experience and persistence of pain and other symptoms, while helping you develop a different relationship with sensations that may otherwise feel threatening. The goal is not to suggest that symptoms are imaginary or "all in your head." Pain and other physical symptoms are always real, even when the mechanisms contributing to them are complex. Trauma-focused treatment is another important part of my approach. When someone has experienced frightening medical events, repeated procedures, prolonged illness, or other types of trauma, the nervous system can remain oriented toward detecting danger even when the immediate threat has passed. Therapy can help you understand these responses, process difficult experiences, and develop a greater sense of safety and confidence. I also place emphasis on helping you move toward what matters to you rather than making symptom elimination the only measure of progress. Depending on your situation, this may involve reducing avoidance, changing patterns of fear and scanning your body for symptoms, improving sleep, increasing meaningful activities, or learning to respond to difficult thoughts, emotions, and physical sensations in new ways. Throughout treatment, I provide a space where you can feel understood without being defined by a diagnosis or medical condition. Your symptoms and experiences are taken seriously, while therapy also creates room to consider new possibilities for how you respond to them. We will develop a clear plan to help you reach your goals and regularly evaluate what is and is not helpful and adjust treatment accordingly.

Dr. Amy Karbasi Practice Description
What makes my practice unique is that I am a trained health and pain psychologist with 15+ years of experience as a psychology expert on healthcare teams in medical systems. This has equipped me with the expertise and tools to understand the medical system, medical conditions and terminology, and medical and psychological treatments for chronic pain and health problems. I have extensive experience and advanced training working with people whose lives have been affected by persistent symptoms, changes in physical functioning, difficult medical experiences, and conditions that may not always be visible or well understood by others. Seeing a psychologist does not mean your symptoms are imagined. Your symptoms are very real, and you've been trying to find a way out for a long time. You feel exhausted and you're looking for a new way forward. I can help you understand the complex connections between your body, brain, nervous system, and emotions, and how addressing all of these areas may bring relief. I will take time to understand you and your goals so that we select treatments that are the right fit for you and your needs. My aim is to help you function better and live the kind of life you want and deserve. One area of focus of my practice is the psychological impact of medical trauma and repeated or difficult medical experiences. This may include traumatic procedures, frightening symptoms, hospitalizations, negative experiences with healthcare providers, prolonged diagnostic journeys, or other experiences that may have left you feeling anxious, mistrustful, or on guard. Therapy can provide a place to process these experiences while reducing fear and developing a stronger sense of safety. Because chronic health conditions can affect nearly every area of life, therapy may address much more than symptoms themselves. Treatment may focus on coping with uncertainty and unpredictability, adjusting to changes in functioning, coping with flare-ups in symptoms, managing difficult emotions, improving relationships, setting boundaries with others, advocating for yourself and your own self-care, reducing avoidance, processing feelings of grief and loss of identity, rebuilding meaningful activities, and developing a life that feels worthwhile even when health challenges are present. I also work with adults experiencing other types of PTSD/trauma, anxiety, insomnia, and other mental health concerns. These difficulties can often occur alongside chronic illness or pain. Therapy is tailored to each person's circumstances, goals, and needs rather than following a one-size-fits-all approach. Working with a psychologist who understands how these problems may intersect and who can address them all in the same person can be ideal. Roan Psychology is a private-pay practice offering secure online therapy to adults located throughout the United States. Virtual therapy allows you to participate from the comfort of your own environment and can be particularly helpful for people whose health conditions, pain, fatigue, or mobility limitations make traveling to an office difficult. I have extensive experience conducting telehealth therapy and find it to be just as effective as in-person therapy in most cases.
