Brielle, New Jersey therapist: Guy J Iacono, licensed clinical social worker
Anxiety

Guy J Iacono

Licensed Clinical Social Worker, LCSW
Anxiety has a way of making everything feel urgent and nothing feel manageable. For many of the men I work with, it shows up as constant overthinking, difficulty making decisions, physical tension, trouble sleeping, or a low-grade sense that something is about to go wrong. It can look like high functioning on the outside while feeling like barely holding it together on the inside. Anxiety is not a character flaw and it is not something you just push through. Left unaddressed, it tends to narrow your life. You start avoiding situations, second-guessing yourself, and spending enormous energy managing something that keeps coming back. In our work together, we focus on understanding what is driving your anxiety, not just managing the symptoms. That means looking at the patterns of thinking that fuel it, the avoidance behaviors that reinforce it, and the underlying beliefs that keep it in place. We build practical skills for tolerating uncertainty, managing the physical experience of anxiety, and making decisions with confidence even when you do not have perfect information. My approach is direct and grounded. I do not believe in spending years analyzing the problem. I believe in building momentum, developing real tools, and helping you function at the level you are capable of. Anxiety does not have to run the show. If you are tired of feeling on edge and ready to do something about it, I invite you to schedule a free 15-minute consultation.

Saint Petersburg, Florida therapist: Michael Zamrin, licensed clinical social worker
Anxiety

Michael Zamrin

Licensed Clinical Social Worker, LCSW, MSW, MA
Anxiety can feel overwhelming, even paralyzing—affecting your thoughts, your body, and your ability to function day to day. I understand how frightening and isolating it can be, and I'm here to help you navigate through it. In our work together, I use evidence-based approaches, primarily Cognitive-Behavioral Therapy (CBT), to help you understand the connection between your thoughts, emotions, and behaviors. I also incorporate mindfulness practices and somatic (body-based) techniques to help you reconnect with your body, calm your nervous system, and stay grounded in the present moment. Together, we’ll identify the patterns that fuel your anxiety and develop practical, integrative strategies to interrupt them. Over time, you’ll build tools to shift unhelpful thinking, regulate physical symptoms, and respond to stress with greater clarity, resilience, and self-compassion. My goal is to help you not just manage anxiety, but regain a sense of control, ease, and well-being in your daily life.

Los Angeles, California therapist: Jayson L. Mystkowski, psychologist
Anxiety

Jayson L. Mystkowski

Psychologist, Ph.D., ABPP
While Cognitive-Behavior Therapy (CBT) is highly effective in the treatment of anxiety disorders (e.g., Panic Disorder, Social Phobia, and Obsessive-Compulsive Disorder), clinicians do see some “return of fear,” or partial relapse, in some patients due to a variety of factors. Over the past two decades, treatment researchers, with whom Dr. Jayson Mystkowski had the pleasure of working with at UCLA for over 10 years, have studied “return of fear” and discovered some key variables that may optimize the effects of learning during CBT for anxiety disorders (Craske et al., 2008). First, evidence suggests that focusing on tolerating fear versus eliminating fear yields better clinical outcomes in the long term. Namely, teaching clients that fear and anxiety are normal feelings, rather than attempting to “down-regulate” such feelings all the time, is more realistic and seems to engender “hardier” clients. Second, helping clients to generate an expectancy that “scary things will not happen,” is very powerful. To do this, it is important for clinicians to create more complex exposure exercises (i.e., tasks in which a client confronts a stimulus of which they are afraid), using multiple feared stimuli instead of one at a time. Then, the lack of a feared outcome becomes particularly surprising and memorable for a client and fear reduction is more potent. Third, increasing the accessibility and retrievability of non-fear memories learned during treatment are powerful factors in mitigating against a return of fear. Craske and colleagues demonstrated that exposure to variations of a feared stimulus, using a random schedule across multiple contexts or situations, is more effective than exposure to the same stimulus, on a predictable schedule, in an unchanging environment. The former paradigm, it is argued, creates stronger non-fear memories that are easier for a client to access when subsequently confronting feared objects or situations outside of the therapy context, than the later scenario. In sum, clinicians have long been aware that some fear or anxiety returns following very successful CBT treatment. As mentioned above, there are some clear, empirically supported ways to modify the therapy we provide to further help clients generalize the gains made in therapy sessions to the real world.
