OCD can be mild to debilitating, and is often missed and treated as anxiety. I've found that OCD and Health Anxiety, as well as Body Focused Repetitive Behaviors (BFRBs, ie. skin picking, hair pulling/tweezing, nail/cuticle biting) can sometimes co-exist together. I'm trained in treating OCD through two of the primary evidence based OCD treatment modalities -Inference-Based Cognitive Behavioral Therapy (I-CBT) and Exposure and Response Prevention (ERP/ExRP). I-CBT is a newer evidence based treatment for OCD that focuses on gaining certainty versus pushing yourself into uncertainty, and is thought to be a gentler approach. There are many types of OCD and the thought-based "invisible" types are the most often missed forms of OCD, often called the "Pure O subtype" meaning that the obsessions/thoughts are there, but that the compulsions are mental and not outwardly visible, as we've come to expect from what we see about OCD in the media/social media. There are many forms of OCD, including Postpartum OCD, Relational (romantic as well as platonic) OCD, Harm OCD, Perfectionism/Just Right OCD, Contamination OCD, Scrupulosity or Religious OCD, Checking OCD, Obsessional OCD, Sexual Orientation OCD, Pedophilia OCD, and Superstitious OCD, among others. I'm trained in treating BFRBs using multiple modalities including, Habit Reversal Training, Acceptance and Commitment Therapy, Comprehensive Model for the Behavioral Treatment of BFRB and DBT, which help to reduce the symptoms of BFRBs.

OCD is interesting in that it could have trauma based origins, physiological origins, including epigenetic, or could be an adaptive response to something that is currently challenging. First is to decipher where the origin is as best as one can, in general an integrative and functional approach may work well, as it could be based in metabolic deficiency, for example. Shared with care: in certain cases, we have witnessed it might evolve in people or communities where power abuse is rampant -- these compulsions can set in as ways to adapt to holding maladaptive and abusive thinking and distracting a person from the inhumanity and incoherence of the abusive thinking they have been groomed to take on. This is not written about from a hegemonic perspective, however decolonizing theory will find these challenges of OCD and even paranoia heightened in people or communities that habituate enacting atrocities. This is not trauma it is an expression of intensive moral injury and benefits from interrupting the moral injury and moving the person into ethical recovery of their humanity. Contrapositively, trauma impactees may show OCD traits as a way of the mind distracting from torture or other forms of recurring harm psychologically (e.g repeated and intensive gaslighting), physically, sexually...etc. Here we would apply a trauma based approach to care along with Havening, Brainspotting and other ways to soothe a nervous system and restore mental networks as best we can/ Please feel free to ask if you have more questions.
