Bethesda, Maryland therapist: Louisa Lombard, licensed professional counselor
OCD

Louisa Lombard

Licensed Professional Counselor, LCPC, LPCC, PPS
OCD can be mild to debilitating, and is often missed and treated as anxiety. I've found that OCD and Health Anxiety, as well as Body Focused Repetitive Behaviors (BFRBs, ie. skin picking, hair pulling/tweezing, nail/cuticle biting) can sometimes co-exist together. I'm trained in treating OCD through two of the primary evidence based OCD treatment modalities -Inference-Based Cognitive Behavioral Therapy (I-CBT) and Exposure and Response Prevention (ERP/ExRP). I-CBT is a newer evidence based treatment for OCD that focuses on gaining certainty versus pushing yourself into uncertainty, and is thought to be a gentler approach. There are many types of OCD and the thought-based "invisible" types are the most often missed forms of OCD, often called the "Pure O subtype" meaning that the obsessions/thoughts are there, but that the compulsions are mental and not outwardly visible, as we've come to expect from what we see about OCD in the media/social media. There are many forms of OCD, including Postpartum OCD, Relational (romantic as well as platonic) OCD, Harm OCD, Perfectionism/Just Right OCD, Contamination OCD, Scrupulosity or Religious OCD, Checking OCD, Obsessional OCD, Sexual Orientation OCD, Pedophilia OCD, and Superstitious OCD, among others. I'm trained in treating BFRBs using multiple modalities including, Habit Reversal Training, Acceptance and Commitment Therapy, Comprehensive Model for the Behavioral Treatment of BFRB and DBT, which help to reduce the symptoms of BFRBs.

Obsessive-Compulsive Disorder (OCD) counseling is a specially designed to help individuals who are struggling with the often debilitating symptoms of OCD. This mental health disorder is characterized by unwanted and intrusive thoughts (obsessions) that lead to repetitive behaviors or mental acts (compulsions). Our counseling services focus on providing varied approaches, such as Cognitive Behavioral Therapy (CBT) and Exposure and Response Prevention (ERP), to reduce the occurrence and impact of OCD symptoms, thereby improving daily functioning and quality of life. Evidence-Based Treatment for OCD * Tailored CBT Techniques: We utilize Cognitive Behavioral Therapy to help clients identify and challenge the distorted thinking patterns that fuel their obsessions and compulsions. * Exposure and Response Prevention: ERP involves gradually exposing clients to their feared objects or ideas (exposures) and teaching them to resist the urge to carry out their usual compulsive acts (response prevention). * Skills Training: Clients are trained in stress management and relaxation techniques to help manage the anxiety that often accompanies OCD. * Mindfulness and Acceptance: Incorporating elements of mindfulness helps clients learn to accept their obsessive thoughts without acting on them compulsively. * Family Involvement: We provide support and education for families of individuals with OCD so that they can better understand the disorder and offer the right kind of support to their loved ones. Goals of OCD Counseling Our OCD counseling services strive to: * Reduce OCD Symptoms: Aim to significantly decrease the frequency and severity of obsessions and compulsions. * Increase Functionality: Help clients regain control over their lives and engage more fully with work, school, and social activities. * Enhance Coping Skills: Equip individuals with tools and strategies to manage OCD symptoms independently and effectively. * Improve Quality of Life: Focus on improving overall well-being, satisfaction, and happiness in life beyond OCD symptoms. * Prevent Relapse: Provide clients with the knowledge and skills to maintain their progress and prevent the recurrence of severe OCD symptoms. Dr. Patricia Webbink

One could say that we all need a bit of Obsessive Compulsiveness to keep our homes and lives organized, but too much of it can result in dysfunctional behaviors like repeatedly checking the stove to see if it was left on, obsessive hand-washing irritating one's skin, etc. The movie “As Good As It Gets” is an example of a man whose total existence is negatively impacted by extreme Obsessive Compulsive Disorder (OCD). Check out my monthly, free, zoom seminars. Please feel free to call for further information and/or a consultation: 301-229-8000.

Exposure and Response Prevention (EX/RP) is the way to go, here. Did you know that there is such as thing as "Pure obsessive" or "Pure O" OCD? Not all OCD involves behavioral compulsions such as counting or hand washing. Some of it is mental compulsions only! Unless you're a true expert, diagnosing OCD can be extremely difficult. Misdiagnoses can be dangerous and costly.

I provide prolonged exposure (PE) therapy for OCD. In this type of treatment, you learn skills to regulate anxiety as it arises in response to environmental triggers, and work on using these skills in session when faced with a compulsion-triggering stimulus. We focus on psychoeducation on obsessional thinking and compulsive behavior patterns, and gradually work with you to use skills instead of engaging in compulsions, so you can have more time and and attention for your relationships, interests, and passions.
