Social anxiety makes us feel like we are a walking target for negative unjust criticisms. Everything feels like a rejection or confirmation of personal shortcomings. Many persons with social anxiety create vivid images in their heads of how they might appear to other people and, needless to say, these images are not very flattering. Many cope by avoiding - giving up on almost all social connections or just that important presentation that could lead to a promotion. Many over-prepare, strategize and become perfectionistic. Many experience what can be best described as retroactive anxiety while analyzing the minute detail of an interaction that might have not gone exactly as planned. Some try to interact but then just freeze and soon after scurry away, feeling no better for the attempt. Cognitive behaviour therapy for social anxiety targets unhelpful assumptions (e.g., "If my dinner party is not perfect, my guests will gossip about me.") as well as attentional processes (e.g., intense scanning of facial expressions). We look at the evidence that keeps these assumptions in place as well as their usefulness. Once we know that the situations that we try to deal with might be uncomfortable but not inherently dangerous, we move towards exposure. We craft behavioural experiments to further test the unhelpful assumptions and create exposure hierarchies for uncomfortable situations that we have no choice but deal with -- you can avoid job interviews or asking someone out but that means that you will be without a job and without a relationship. Each more difficult interaction serves as a learning step. We learn better skills to handle conflict or embarrassment. We learn that although uncomfortable such interactions are manageable leading to eventual decrease in anxiety sensations. Most of all, we build an ability to remain coherent under pressure and despite being judged by others. Some of these exposures occur in real life as you slowly learn to face your challenges, yet the bulk of them occurs in my office as we practice conversation skills, assertive defense of the self or engage in high voltage simulated arguments to inoculate you against the unfairness that unfortunately exists in the world around us. Most of all, we learn to not to overthink.
