Anxiety takes on many shapes, from the physically overwhelming intensity of a panic attack to the never-ending questions that characterize generalized anxiety disorder. Whatever it's form, anxiety is maintained by the interaction between our anxious predictions (thoughts), attempts to escape or avoid our triggers (behaviours) and an almost chronic preoccupation with anxiety related symptoms (attention). Canadian Clinical Practice Guidelines recommend cognitive behaviour therapy (CBT) as the first line of treatment for anxiety disorders. CBT helps us ensure that our thoughts reflect our reality and not our apprehensions, that our behaviours match our goals, and that our attention is directed towards what is effective instead of wandering way off into the future. Exposure-based interventions are central to CBT for anxiety. Exposure exercises teach us how to approach what we fear in a safe manner. After all the things that we avoid are often the ones that are the most important, like having that difficult conversation or studying for an exam. Procrastination has far more in common with anxiety than with laziness. Exposure-response prevention exercises help us eliminate needless precautionary behaviours like monitoring our physical states to predict the next panic attack or seeking constant emotional reassurance from people around us.
